Please fill in the family members that you would like to include in the obituary in the form below. You will also be asked for some other pertinent information, which will help us create an obituary.

  • focus your efforts on proper spelling and ensuring that everyone that should be included is included
  • you will have the opportunity to customize and personalize the obituary at our arrangement conference
  • you will receive an email including all of the information you submit immediately after completing this form
  • ***Please include any family members that have predeceased your loved one with a (D) before their name. For example: John Doe of Crediton, (D) Mike and Sarah Doe of London, and (D) Jane Smith
